Choline Donors
Choline donors are the supplements used to supply the precursor for acetylcholine synthesis. They form the substrate row of the cholinergic stack in the Amplification Framework, the role that piracetam does not fill.
Choline is an essential nutrient in its own right — required for acetylcholine, for phosphatidylcholine in every cell membrane, and as a methyl donor via betaine. Most people do not reach the adequate intake from diet.
The forms, and why they differ
- Alpha-GPC (L-alpha-glycerylphosphorylcholine) — crosses the blood-brain barrier readily; the highest choline yield by weight of the common forms (~40%). A prescription medicine in some European countries for cognitive indications.
- Citicoline (CDP-choline) — supplies choline and cytidine, which converts to uridine, a separate pathway involved in phospholipid synthesis. Better human trial evidence than alpha-GPC, particularly post-stroke and in vascular cognitive impairment.
- Choline bitartrate — cheap, poor CNS penetration. Adequate as a nutrient, largely useless for the nootropic purpose it is sold for.
- Phosphatidylcholine / lecithin — food-form, slow, low yield.
- Centrophenoxine — a DMAE ester, older literature, associated with lipofuscin clearance.
The practical point: these are not interchangeable. Bitartrate and alpha-GPC differ both in choline yield and in whether they reach the brain, and the price difference between them is the reason bitartrate is what appears in most combination products.
The TMAO question
Gut bacteria convert choline to TMA, which the liver oxidises to TMAO — a metabolite associated in observational studies with cardiovascular risk. A 2021 randomised trial found alpha-GPC raised TMAO more than an equivalent dose of choline bitartrate. Whether TMAO is causal or a marker remains genuinely contested, and dietary choline from eggs and meat produces it too.
What is worth flagging honestly: this is the one open safety question about the most-recommended form, and it is rarely mentioned in the nootropic literature that recommends it. It is not a reason to avoid choline; it is a reason not to treat alpha-GPC as free of consequence at high chronic doses.
Cautions
- Excess choline produces fishy body odour, sweating, hypotension and GI upset — the trimethylamine again.
- Cholinergic overshoot — headache, depression-like flattening, brain fog — is commonly reported when choline is stacked with a racetam at high dose, and is the mirror image of the deficiency the pairing was meant to correct. More is reliably not better here.
- Relevant in depression: cholinergic tone is implicated, and some people reliably feel worse.
- Interacts with anticholinergic and cholinesterase-inhibitor medication.
