Black Pepper

Black pepper (Piper nigrum) is the most-traded spice in the world and, less obviously, one of the more pharmacologically dense things in an ordinary kitchen. It contributes four distinct kinds of activity to the endocannabinoid framework, which is more than any other single plant in the Cannabinoid Oilahuasca inventory.

The four activities

Enzyme inhibition, transport inhibition, receptor agonism and absorption enhancement, from one plant. That convergence is the reason the family is treated as a unit in this corpus rather than as four unrelated ingredients.

Piperine and the bioavailability question

Piperine raises the systemic exposure of a long list of co-administered compounds — curcumin is the famous case, with reported increases of roughly twentyfold — by two mechanisms: inhibition of CYP3A4 and of P-glycoprotein efflux, and inhibition of intestinal and hepatic glucuronidation.

This is the most dangerous thing on this page, and it is sold as a benefit. A compound that raises the bioavailability of whatever is taken with it raises the bioavailability of prescription medication too. CYP3A4 handles a very large share of clinical drugs; P-gp governs the absorption of many more. Piperine is added to supplement formulations specifically for this effect, at doses well above culinary exposure, and is routinely taken by people also on medication.

Where an interaction matters — anticonvulsants, immunosuppressants, anticoagulants, antiretrovirals, chemotherapeutics, cardiac drugs with narrow therapeutic windows — a piperine-containing supplement is a pharmacokinetic intervention, regardless of what the label says it is for.
